PPO plans: Blue Cross, Blue Shield, Aetna, United Healthcare are among
several accepted preferred provider plans. We do not participate in HMO
or Medi-Cal.
Patients are responsible for providing us with accurate and
timely insurance information. Photocopies of your insurance card and
driver's license are required at each visit. Please understand that we
are obligated to collect co-pays mandated by your insurance carriers and
discounts are disallowed to select patients.
Billing is a one-time courtesy
provided by our office. Addditional fees may apply for assistance in billing
for outstanding balances. At your visit, expect to pay your co-pay in full.
Cash, check and credit cards are accepted.
If payment is not received within
60 days of electronic submission, please understand that you are responsible
for the balance. A detailed explanation of benefits covered and payment due
will be provided.
Our malpractice coverage is through the cooperative of
American Physicians, CAP-MPT. Patients are required to sign an arbitration
agreement prior to consultation and examination.
Effective January 2008 we will no longer accept Medicare.
